Buy a cheap shower door to get the fittings you need then get a piece of glass cut and edged to the size and shape you needed and make the fittings fit. It'll have to be toughened for safety reasons. The glazing places that fix broken glass and do 24hr call-outs tend to be very expensive. Try to find a small sealed-unit maker who operates at the competitive end of the market.
