Might be the Hardy Spicer type as fitted to the Mini Cooper S and automatics.

A machine shop would cut four blocks of steel and put holes through them to your measurements fairly cheaply.

You would have them made a touch undersize so that alignment with the shaft can be set using shims under them. These can be cut with tin snips U shaped from any flat tins, like biscuit tins.
